چکیده
Rev Esp Cardiol. 2005;58(9):1003-6 1003 Despite the advances in surgical techniques, cardiopulmonary bypass circuits and the medical treatment of patients subjected to cardiac surgery, perioperative complications involving the nervous system continue to develop and the underlying mechanisms are sometimes poorly understood. Neurological complications (NC) in these patients can affect the brain, spinal cord and peripheral nerves. However, the involvement of the central nervous system, and cerebrovascular involvement in particular, is the most common adverse event. It is associated with high rates of morbidity and mortality, longer hospital stays and greater utilization of health care resources, in addition to the severe financial and social distress that these complications bring upon the patients and their families. The incidence of NC following cardiac surgery depends on the methodology employed for its diagnosis, on the terminology utilized and on the application of a prospective or retrospective approach in its evaluation. In a recent report assessing the type of surgery performed,1 NC (transient ischemic attack or stroke) presented in 1.7% of the patients undergoing coronary revascularization, in 3.6% of those having single valve replacement, in 3.3% of those subjected to both procedures and in 6.7% of those undergoing multiple valve replacement. These values agree with those reported for previous series, which indicated a greater number of NC after valve replacement than after coronary revascularization. In fact, in some series of patients subjected to valve replacement, an incidence of stroke of up to 16% has been observed. ED I TO R I A L S

Inter-Operative Time between Neurosurgery and Cardiac Surgery for Native Valve Endocarditis
Introduction: Infective endocarditis is well known to produce neurologic complications from septic emboli, which mandates a multidisciplinary approach between neurosurgery and cardiac surgery. However, literature is not clear as to the optimal interval length between neurosurgery and cardiac surgery.Presentation of Cases: The first case involves a 60-year-old woman with significant past medical...
متن کاملSex differences in neurological outcomes and mortality after cardiac surgery: a society of thoracic surgery national database report.
BACKGROUND The purpose of this study was to evaluate whether women undergoing cardiac surgery are more likely to suffer neurological complications than men and whether these complications could explain, at least in part, their higher perioperative mortality. METHODS AND RESULTS The Society of Thoracic Surgery National Cardiac Surgery Database was examined for the years 1996 and 1997 to determ...
متن کاملNeurological Study of Radial Nerve Conduction During Endoscopic Radial Artery Harvesting:An Intra‐Operative Evaluation
Endoscopic radial artery harvesting (ERAH) is a feasible and attractive minimally invasive approach for conduit procurement, however there have been concerns about a potential neurological damage occurring at the harvest limb site secondary to injury of the radial nerve during endoscopic harvesting. We present a case of ERAH in which we evaluated intraoperatively the characteristics of radial n...
متن کاملThe effects of magnesium supplementation on the incidence of cardiac complications after pediatric cardiac surgery
Background and Objective: The present study aimed to assess the role of magnesium sulfate (MgSO4) sup-plementation after cardiac surgery in pediatric patients, evaluating the incidence of cardiac complications, such as arrhythmia. Methods: In this study, 105 children scheduled for elective cardiac surgery were randomly divided into 3 groups. The first group, as the placebo group, received sali...

Although mortality rate after cardiac surgery has been drastically reduced, neurological complications remain a significant problem. Several etiologic factors have been proposed, including previous unrecognized neurological abnormality, embolic events, hypoxic insult, low cardiac output, systemic inflammatory response, and altered cerebral blood flow (CBF) and metabolism. Cerebral ischemia can ...
